This is the CR5 Radius Fretless, a 5-string fretless bass from NS Design.

Designed by Ned Steinberger, known for his innovative design concepts, this model boasts an exquisite fit and exceptional playability.Manufactured in the Czech Republic with a curved body shape, it features a flamed maple top and maple body, an ebony fingerboard, and is equipped with an EMG humbucker pickup and a built-in piezo pickup. It includes 1 Volume control, Treble and Bass controls, a Pickup Blend control, a 3-way Pickup Switch, a 3-way Piezo EQ Switch, and an NS Design Hipshot bridge.

This model boasts a wide variety of sounds thanks to the combination of the piezo and active humbucker.

There are signs of use (scratches, play lines, picking marks, string marks on the fingerboard, tarnish, pitting, or rust on metal parts and screws, and other cosmetic damage).

It was set up upon arrival (cleaning, truss rod adjustment, octave adjustment, and string height adjustment). The truss rod can be adjusted in both directions and has been tested to operate smoothly within a range of ±60°, with no issues found.
